Our primary care physicians’ offices are located on the second level of the building. Upon entrance, you may take the elevator or stairs up to the second floor. There is a single desk for check in for all the physicians in the group.
If you are transferring care to our clinic from another physician please call your previous physician’s office to sign a records release form PRIOR to your appointment. This ensures that your new primary care doctor will have your previous records during your appointment.
The following is a list of items to bring to your first appointment:
Completed New Patient Forms (listed below)
- List of medications or prescription bottles
- List of allergies
- Drivers License
- Insurance Cards
- All payments or co-payments due at the time of service
You may be asked to go to our in-house Lab and X-ray prior to your appointment.
A nurse from your physician’s office will escort you to the exam room for your visit.

To expedite the check-in process, please complete the following forms prior to your arrival to our clinic.:
Co-payment and/or Past Due Balances are due at time of service.
We accept cash, checks and most major credit cards.
